- Step 1: Call us and get the F.A.C.T.S about Cloverleaf Care and what value they bring to you or your loved one. We are available 24/7 to answer any phone inquiries. At the end of this step you will have a broad idea about our home care services portfolio and various service plans.
- Step 2: Assessment. This is a no obligation FREE consultation with our Care Team. We will design a plan of care customized to meet your unique needs. For your convenience we will meet you in your home at a time that works best for you. Most, if not all, of your questions regarding our home care services, including our unique Total Care Solution, should have been fully answered at this point. We will even answer questions you may not have thought of yet! At the end of this step you will have a detailed understanding about our home care services as they apply to your specific situation and what will be the daily plan of care.
- Step 3: Start Services. A Home Care Aide (Caregiver) is matched to your needs. Our Care Team, who you will have already met during the assessment step, will introduce you to the Home Care Aide and do an on site orientation to begin services. We pride ourselves on our quick response and are able to start home care services at very short notice, even on the same day. We will review with you what you should expect from our Home Care Aides and how you can access daily support from our office.
- Step 4: Total Care. Cloverleaf Care’s Total Care Solution provides a solution to your continued home care needs. We routinely reassess your care needs and adjust your plan of care. We communicate with you and your family regularly as to your health and well being. We provide ongoing support to you and your family through our industry leading Home Care Case Management and Home Care Supportive Services - both part of our Total Care Solution.
